Why does he need to have his system flushed after receiving the Methotrexate?
Andy wrote: Chemo is great for us when it is in the cancer cells. Pretty much everywhere else in the body, however, it is toxic. So, once it goes through the blood stream and starts moving into the kidneys and bladder, then you want to keep it moving along.
That's why you are supposed to stay well hydrated with chemo. While I haven't had methotrexate, I imagine that something very similar is the issue. I looked over the methotrexate prescribing info and it looks like there can be a lot of variability in how long it takes to clear out of the system and there are things they can help to flush it out and prevent damage to organs. How much of the 4 days is the infusion and then how much is the flushing time?

"The neurotoxicity commonly manifests as ... constipation, and peripheral neuropathy which is predominantly sensory in nature. The early symptoms include numbness and tingling of hands and feet which is accompanied by loss of deep tendon reflexes. In its more severe form, muscle weakness develops which is more marked in distal muscles of the hands and feet. Other manifestations of neurotoxicity include ocular palsies, hoarseness of voice, and autonomic neuropathy in the form of postural hypotension and atony of the urinary bladder. The neurotoxicity is dose related and cumulative with repeated dosage such that the drug therapy has to be stopped after a cumulative dose of 30 to 50 mg. The neurotoxicity is usually reversible on interruption of the therapy, but the recovery is slow and takes several months. There are no specific antidotes which have established usefulness against neurotoxicity." PMID

Contact your doctor immediately, if
Fever over 100 degrees - indicating possible infection
Contact your doctor as soon as possible, if
Lower back or side pain - indicating possible kidney problems
Swelling of feet or lower legs
Gout-like joint pain
Changes in eye sight, blurry vision, or drooping eyelid
Trouble walking or climbing stairs
Severe constipation or stomach cramps
Difficulty urinating, or frequent need to urinate - bed-wetting
Contact your doctor when needed, if these symptoms continue to bother you
Uncontrolled nausea, vomiting, constipation or diarrhea
Easy bruising or bleeding
Severe jaw pain or headache
Redness, swelling, pain or sores where the needle was placed
Redness, swelling, pain or sores on your lips, tongue, mouth or throat
